Schedule: PRN/Days
Shift: Day 4-8 visits weekly
Location: AdventHealth– Hinsdale Home Health
The role you’ll contribute:
Evaluates and treats home health patients requiring speech-language therapy
The value you’ll bring to the team:
Performs therapy interventions utilizing standard therapy techniques and skills as appropriate for the condition of the patient.
Educates and counsels patient and family regarding treatment plans.
Maintains clinical records and follows performance improvement recommendations.
Plans and/or assists with patient discharge from home health speech therapy services. Provides information regarding appropriate support programs.
Qualifications
The expertise and experiences you’ll need to succeed:
Bachelor's degree required.
BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or
job transfer date.
Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior
to hire date or job transfer date.
Speech-Language Pathologist credentialed from the Illinois Department of Financial and Professional
Regulation obtained prior to hire date or job transfer date.
