Licensed Master Social Worker - Comprehensive Care Clinic

Your Role:
The responsibility of this social worker is ensuring that patients have access to care primarily by enrolling and maintaining coverage in the federal Ryan White HIV/AIDS Program. This role requires a great deal of paperwork, attention to detail, and ability to learn about complex processes and how they interact. This social worker will also conduct the psychosocial assessment, orientation, and enrollment in Ryan White services (if applicable) for patients new to the clinic either because of a change in provider or a new HIV diagnosis. In this capacity, the social worker will have the opportunity to provide education and brief supportive therapy.
Department Summary:
The Vanderbilt Comprehensive Care Clinic (VCCC), a nationally recognized HIV Center of Excellence and a certified Primary Care Medical Home, is located at Vanderbilt Health's One Hundred Oaks campus in Nashville, Tennessee. The V-CCC provides care for more than 3,700 active outpatients living with HIV from Tennessee and adjacent states. The VCCC has on-site psychiatric, adherence counseling, social work, and nutrition services. Patients with HIV/AIDS receive quality, comprehensive HIV care, regardless of their ability to pay.
We are seeking qualified social workers who have a heart for caring for marginalized patients, who can work in a fast-paced environment, and enjoy a collaborative team-based work setting.
Shift Hours:
Monday - Friday / 8:00 a.m. - 5:00 p.m.

KEY RESPONSIBILITIES:
- Maintains professional documentation which meets department and VUMC standards as it relates to compliance.
- Provides brief targeted interventions for identified clients.
- Completes psychosocial assessment and assists team with the development of the plan of care.
- Provides education and or consultation to clients and families, treatment team and community.
- Contributes to the organization by actively participating in process improvement initiatives and shared decision making.

TECHNICAL CAPABILITIES:
RISK MANAGEMENT (NOVICE):- Possesses sufficient fundamental proficiency to successfully demonstrate risk management of moderate difficulty. Capable of conducting risk analysis as part of a task to address problems including determining risk factors, probability, impact, early detection and prevention. Able to prepare simple contingency plans and alternatives.
PATIENT ASSESSMENT (NOVICE):- Possesses sufficient fundamental proficiency to successfully demonstrate patient assessments in practical applications of moderate difficulty.
